Course Substitution or Waiver Form

(This form is used to substitute one course for another and requires written justification. If using a substitution or waiver to satisfy a degree requirement, additional credit hours may be required to meet the minimum total hours required of a degree.)

Graduate Transfer Credit Form

(This form is used for graduate courses completed at another institution prior to applying to Shepherd.)

Accelerated 4+1 Bachelor’s/Master’s Programs – M.B.A., M.A.T., APST

(This form is used for undergraduate students who were accepted into the accelerated bachelor’s/master’s program – 4+1.)
