Authorization for Release of Information
Shepherd University is committed to a student’s right to privacy and confidentially. Student files are kept separate from academic files and are stored in a secure location within the Disability Support Services office. Information contained in a student’s record is confidential and protected as an educational record under the Family Educational Rights and Privacy Act (FERPA). Disability Student Services will not provide a student’s information to parties outside of the university without the student’s express written consent.
Information held by the Office of Disability Student Services may be shared with Shepherd University faculty and staff on a limited, need-to-know basis, in order to facilitate the provision of accommodations and services, as indicated on the Accommodation Request Form signature page. Student information is not shared with parents or spouses unless the student gives explicit written permission through the Authorization for Release of Information Form. Even with permission, student information is shared on a limited, need-to-know basis at the discretion of the Coordinator.
