MONTGOMERY GI BILL (MGIB)

If you have not applied for your MGIB, Post 9/11 GI Bill, or Yellow Ribbon Program benefits:

Active Duty and Reserve Benefits (Chapters 30, 33, 1606 and 1607)

* Complete the application for VA Education Benefits at: http://vabenefits.vba.va.gov/vonapp/main.asp

* Complete and submit the Coordinator Card to the Shepherd University Veterans Certifying Official
o You can e-mail this card to mwalling@shepherd.edu, Shepherd's VA Certifying Official (include in your e-mail that you have applied for your benefits)
o You can fax it to 304-876-5137 to the attention of Mary Beth Walling (included a statement that you have applied for your benefits)
o You can drop it by the Student Affairs Office, First Floor, Student Center.

* Once you begin classes, bring a copy of your DD214 (member-4) to Mary Beth Walling, Student Affairs Office, First Floor, Student Center.

* At the end of each month you will need to contact the VA to Certify Your Attendance. You can contact them in one of two ways:
o Call 1-877-823-2378 or
o Go to www.gibill.va.gov

Survivors' and Dependents' Educational Assistance Program (Chapter 35)

* Complete the application for VA Education Benefits at: http://vabenefits.vba.va.gov/vonapp/main.asp

* Once approved the Veterans Administration will send you a "Certificate of Eligibility". You will need to bring a copy of your certificate to Mary Beth Walling, Student Affairs Office, First Floor, Student Center.

* Complete and submit the Coordinator Card to Mary Beth Walling.

VA Vocational Rehabilitation (Chapter 31):

* Veterans need to make application with a VA Vocational Rehabilitation Counselor (Jeffrey.Stoddard@va.gov) (304-263-0811, ext. 4677) and have the necessary disability rating.

* Once the appropriate disability rating is made, the VA will send Shepherd University an Authorization Form 1905.

* The student will then need to complete and submit to Mary Beth Walling a Coordinator Card.

If you have used your GI Bill at another institution or are changing majors:

Active Duty and Reserve Benefits (Chapters 30, 33, 1606 and 1607 or Yellow Ribbon Program)

* Complete form 22-1995 Request for Change of Program or Place of Training.

* Complete the Coordinator Card.

* Submit both forms to Mary Beth Walling, mwalling@shepherd.edu.

Survivors' and Dependents' Educational Assistance (Chapter 35)

* Complete form 22-5495 Change of Place of Training or Major and submit to Mary Beth Walling at mwalling@shepherd.edu. You can also bring the completed form to the Student Affairs Office, First Floor, Student Center.

* You can also meet with Mary Beth Walling at the Student Affairs Office, First Floor, Student Center, to complete this paperwork.
