Registrar Forms on the Web

Tim Haines from External Affairs recently met with the Office of the Registrar staff during an annual retreat and training day. The goal: teach us how to use software that can create Web-based interactive versions of our routine paper forms.

We already have several of our forms on our Web site, but they must still be printed and filled out by hand, then faxed or mailed in. Student handwriting is often difficult to make out, especially after being faxed, and crucial information is sometimes left out entirely--so while these forms are a convenience to students, it is not always easy to process them once they arrive. By making forms that the student can fill out online, we help ensure that their information is complete and legible.

Our first project was a big one, and the form students usually dread the most:  the Admissions and Credits petition http://www.shepherd.edu/register/pdf/A&C_Petition.pdf form!  By making this a Web-based interactive form, we were able to include it in the e-mail we send to students when they are dropped for non-payment. We hope that by pushing the form out to students in this situation, we will help speed their return to the class rolls--which is good for the institution, too.  The online form includes instructions to student and advisor, as well as a page for students to write a clear statement to the committee.

Each area of the office has identified one or two often-used forms that we will then similarly "webify" and make available to students and staff through our Web site. Many thanks to Tim for spending the morning with us, and for his continuing support on this project.
