Health Requirements Verification Sheet
Upon entrance into the nursing program submit a completed Health Requirements Verification sheet to the Department of Nursing Education prior to July 1 if accepted into the program for fall semester and prior to December 1 if accepted into the program for the spring semester. This sheet must be completed by a physician or nurse practitioner and requires a physical examination, immunization verification and 2-step PPD test results.

CPR Certification
Department of Nursing Education accepts only American Heart Association Healthcare Provider CPR. Submit a current copy (front and back) of your CPR certification card to the Department of Nursing Education  by date designated on New Student Checklist..

 

 

 

 

 

 

 

 

Criminal Background Check
A fingerprint card will be mailed upon your acceptance into the nursing program. Fingerprinting is done at Shepherd University by the campus police. Call 304.876.5374 for an appointment. Submit the completed fingerprint card and criminal background check fee at the mandatory nursing assembly. Print and follow instructions carefully.

ATI Testing Fee
Nursing students are required to take Assessment Technologies Inc. (ATI) achievement tests while progressing through the nursing program. Near the end of the program, ATI comprehensive tests will be administered to assess students’ readiness to take the NCLEX. The student is responsible for payment of fees associated with this testing program. Reference New Student Checklist for details

Student Handbook and Policy Manual
Adherence to the policies and procedures of the Department of Nursing Education are special requirements of all nursing majors.  Hard copies of both the Student Handbook and Policy Manual are given to each new student at the nursing assembly and are also available on the Student Resources page of this website. Be prepared and ready for the new semester by looking over these books to find answers to many questions you will have as a new student.
