This form is used to change Degree, Major, Minor, Concentration, Teaching
Field and/or Academic Advisor. |

General Studies
Computer Literacy: This form documents computer literacy. It must be
submitted to the Registrar prior to or with the Senior Evaluation Form. |

Permit To Withdraw From Class:
This form is obtained from the student's advisor when a student withdraws from
one or more (but not all) classes after the first five days of classes.
This form is submitted to the Office of the Registrar. Complete
withdrawal from Shepherd University requires a separate form available in the
Office of the Registrar |
