Students seeking accommodations must provide documentation of their needs. Each student’s documentation requirements will differ since accommodations are provided on an individualized basis. Students should never delay meeting with the Disability Support Services office or requesting accommodations for fear of a lack of documentation.
Any third party documentation which may be required must be printed on official letterhead and signed by a qualified, licensed professional in the appropriate area of expertise and should:
- Be current, within the last five years prior to the time of enrollment. (Documentation for disabilities of a permanent unchanging nature need not fall into this time frame.)
- Describe functional limitations as a result of their abilities and disability as well as any barriers to access that these limitations might cause.
- Provide evidence that their abilities and disability significantly impact a major life function that could potentially impact performance and success in the college environment.
- Identify and describe the current need for specific, suggested accommodations relevant to their performance in the college environment.
