Authorization for Release of Information
Shepherd University is committed to a student’s right to privacy and confidentially. Student disability files are kept separate from academic files and are stored in a secure location within the Disability Support Services office. Information contained in a student’s disability record is confidential and protected as an educational record under the Family Educational Rights and Privacy Act (FERPA). Disability Support Services will not provide a student’s information to parties outside of the university without the student’s written consent.
Disability-related information may be shared with Shepherd University faculty and staff on a limited, need-to-know basis in order to facilitate the provisions of accommodations and services, as indicated on the Accommodation Request Form signature page. Student information is not shared with parents unless the students give explicit permission through the Authorization for Release of Information Form. Even with permission, student information is shared on a limited, need-to-know basis t the discretion of the coordinator.
